Winning Wheels has developed basic guidelines for the determination of eligibility and continued placement. These guidelines have been established to assist the admissions review team in making placement decisions.
An application is obtained from the Director of Admissions or can be downloaded by clicking here. More information about how to view this document can be found at the bottom of this page. Prior to a determination of eligibility for placement, a completed application or necessary medical documents must be submitted for an evaluation to be performed. The final decision of the admissions review team is contingent on the ability of the existing programs and services to adequately meet the identified needs of the prospective client.
General Admissions Criteria
- Individuals who require rehabilitative services due to a neurological impairment (i.e. Brain Injury, CVA, spinal cord injury, brain tumor, aneurysm).
- Individuals who are eighteen (18) years of age or older.
- Individuals who require intermediate or skilled nursing care
- Individuals who, in most cases, may be able to graduate to other less structured living situations upon completion of the facility's rehabilitation program. It is recognized that some individuals, due to the severity of their disability or the degenerative nature of their medical condition, may not be eligible for community living. The expectation for these individuals is the learning of skills or the stabilization of their individual disability, where a less intensive level of nursing care may be possible.

Upon receipt of the completed preliminary application or medical records, a personal interview should be scheduled with the prospective admittee and family or appropriate significant others. The purpose of the interview is not only to aid in gathering information for admission purposes, but also to afford the patient the opportunity to tour the facility and ask questions. Special consideration will be given to the individual's goals and expectations and to the suitability of available services.

Just a reminder to avoid delay: Please DO NOT send the "Release of Information" form back to us at Winning Wheels. After you have completed the Medical Record Release forms, you MUST send them to the appropriate locale, and they then will send us the Medical Records that you ave requested.
